COPD Grouping Standards

Based on a COPD patient’s symptoms, spirometry results, and exacerbation risk, per the international GOLD standard, doctors classify a patient’s COPD into one of four groups:

  1. Group A (GOLD 1 or 2): Very mild symptoms. FEV-1 is 80% or more. One or less flare-ups in the past year and no hospitalization.
  1. Group B (GOLD 1 or 2): FEV-1 is between 50% and 80%. More symptoms than people in Group A. This is the stage where most COPD patients see their doctor for occurrences of coughing, wheezing, and shortness of breath. Possibility of  a major flare up in the past year, but no hospitalizations due to COPD symptoms.
  2. Group C (GOLD 3 or 4): Air flow in and out of the patient’s lungs is severely limited. FEV-1 is between 30% and 50%. More than two flare-ups in the past year, and at least one hospitalization. 
  3. Group D (GOLD 3 or 4): Patient finds breathing immensely difficult. Minimum two or more flare ups in the past year, and one or more hospitalizations. Doctors call this “end-stage” COPD which means minimal lung function. Any new flare-ups and exacerbations could be life-threatening.

COPD grouping is integral to the treatment and symptom management process. It allows physicians and the patient’s healthcare team to enact appropriate protocols and proactively manage symptoms while educating the patient and their family about risk factors, lifestyle changes, and next steps.
